Preparing for your appointment

Prepare well for your appointment at our centre. Bring your insurance card and valid proof of identity with you. You can read about what to expect here

Bring your insurance card and valid proof of identity with you. Proof of identity which has expired is not valid.

You can prove your identity at all the CCN centres by means of your:

  • Passport
  • ID card
  • Driving licence
  • Dutch residence permit document

Furthermore:

  • Think about any questions you want to ask.
  • Bring a list of your medication with you or the boxes they come in.
  • You do not need to have fasted before the tests unless you have specifically been told so.
  • If anything is unclear or if there is anything you do not understand, ask for an explanation.
  • Follow your healthcare provider’s instructions and advice.

  4. If there is anything you do not understand, ask for an explanation
  5. If you need to make a difficult decision, ask for time to think about it
  6. Carefully follow the instructions/advice of your healthcare provider

Cardiologie Centra Nederland may not pass your data on to others outside Cardiologie Centra Nederland without your permission, unless it has a statutory duty to do so, for example supplying data to the health insurer pursuant to the Dutch Healthcare Insurance Act. We treat your data with care.

The rules which apply to this have been incorporated into Cardiologie Centra Nederland’s privacy regulation.
